To update a facility name in Passport, navigate to the facility's dashboard. If using a corporate dashboard, toggle to the desired facility first. From the menu bar, select Dashboard > Facility Profile. On the Facility Profile page, you can update the facility's name, address, city, state, and ProviderTrust code. Note that changing the state impacts credential issuers for license requirements. Additionally, existing units tied to the facility can be reviewed and edited for name, ProviderTrust code, or state details directly on this page.
You can also update a facility name using a facility file (Create Facility Data File).
Important note: If you have license requirements, the state will affect the issuer for your missing credentials.
ProviderTrust Code refers to the code used on your Provider File if you need to change this code, you would also update it on this page.
